POLICY

1. There are no refunds given once appointment is scheduled.

2. Must secure your appointment with a card on file and a deposit for IV HYDRATION/BOTOX SERVICES. Without a deposit, your appointment is not secure and can be taken by someone who does leave a deposit.

3. All deposits go toward the cost of your service.

4. In the event of rescheduling your appointment, your initial deposit will be used for the newly scheduled appointment. No new deposit is needed. (ONLY 2 rescheduled appointments allowed )

5. In the event of a NO CALL, NO SHOW or cancellation less than 24hours in advance; your deposit will not be refunded. A $50.00 cancellation fee will be charged as well. You must schedule a new appointment with a new deposit.

6. Please do not forget to complete the MEDICAL CLEARANCE and CONSENT FORMS BEFORE your appointment time.
